BILLING ACKNOWLEDGEMENT
Dear Patient,
We would like to take this opportunity to explain the billing and out of pocket expenses
for home sleep testing at United Sleep Apnea Services. Our fee for service model
means that each sleep diagnostics procedure costs $350 or less, depending on your
insurance benefits.
We conduct more than one night of testing as it provides us with a better
understanding of your sleep architecture, allowing our sleep physicians to conduct a
more comprehensive evaluation and interpretation. You will only be responsible for
paying copays and deductibles for the diagnostics night used for interpretation, and the
total amount you will pay will not exceed $350 for that night used. If any of the
additional nights are not used for interpretation, you will not be responsible for any
copay or deductible charges for those nights, and we will only courtesy bill those nights
that the unit was used.
Please note that your Explanation of Benefits may show an invoice price and indicate
that you owe the remaining balance. However, United Sleep Apnea Services does not
balance bill, and you will never be asked to pay more than $350 total out of pocket
based on your remaining deductible at the time of service and any copay or
coinsurance owed. We have provided an Insurance Consent Form within this packet
that is an agreement and will indicate that you are not liable for more than the $350 for
your records. Please ensure you keep this for your records.
